The Disparities in Transition of Care after Acute Stroke Hospitalization

The Transition of Care Stroke Disparity Study is an investigator-initiated, prospective, multi-center observational stroke case study. The objective of this study is to identify race-ethnic and sex disparities in hospital-to-home transition of stroke care and stroke outcomes, and to develop effective hospital-initiated system level initiatives to reduce these disparities, improve stroke outcomes, and reduce readmissions. The overarching goal of the study is to eliminate disparities in hospital-to-home transition of stroke care and in adverse stroke outcomes including readmissions, stroke recurrence, and death. A core outcome of our research is to determine the contributions of Social Determinants of Health to identify disparities in transitions of care and stroke outcomes, and to incorporate these results in current or new educational initiatives. This study will address the important public health issue of disparities in the stroke transition of care, set the stage for future large interventions to reduce disparities in transition of stroke care and improve stroke outcomes, and form a strong partnership between health care providers, patients, communities and other stakeholders as a powerful strategy to optimize transition of stroke care for the diverse population of stroke patients in Florida.
