There is no charge for current residents/fellows or for residents/fellows that have completed training within the past 2 years.
After completing and submitting the credit card form, you will see a payment confirmation page and receive a confirmation email. Please keep this "receipt" for your records.
Completed verifications will be returned 1-2 weeks after payment is received.

The Department of Orthopaedics does not have, under any circumstances, access to your credit card information. Your credit card information is used to process your verification fee payment and will not be used for any other purposes. Credit card information is transmitted through, but is not stored, on a web-enabled server. Please be aware your credit card will be billed from our third-party service provider Nelnet Campus Commerce, USA.
