The Geriatric Medicine Fellowship is under the auspices of the University of Miami and JHS consortium, with its directorship and support deriving principally from the Division of Geriatrics and Palliative Medicine in the Department of Medicine. The division participates fully in the educational affairs of the Department of Medicine. The faculty of the division are responsible for staffing the geriatrics programs at various sites in south Florida, including: 1) Miami VA Medical Center; 2) Jackson Memorial Hospital; 3) MorseLife in West Palm Beach, Florida; 4) Miami Jewish Health Systems; and 5) Leon Medical Centers.
We strive to elucidate and apply the biopsychosocial aging model to improve health and independence (quality of life) for older persons. We achieve these goals through understanding, applying, teaching, innovating, and promulgating interprofessional approaches to the problems of the older person in the modern world.
Our holistic approach integrates faculty from the entire spectrum of academic disciplines, ranging from molecular genetics to health-policy development. This team of professionals works in pursuit of the following specific aims:
- To develop, promote, and sustain interprofessional models for delivering, teaching, and innovating elder/senior care.
- To provide basic training in aging, longevity, gerontology, and geriatrics to health professions students, house staff, and practitioners and to demonstrate the reduction of theory to practice in models of senior care. Whenever possible, emphasis is given to the biopsychosocial aging model and the physical and psychosocial aspects of older patients at various stages of function and placement.
- To conduct research aimed at improving life quality and/or longevity of aging persons.
The program offers training in the inpatient and outpatient setting, covering all geriatric settings. Acute Care and Care Transitions, Long Term Care – Nursing home, outpatient care following the Patient Centered Medical Home Model (community) and Patient Aligned Care Team (VHA). The program also offers training for consultation services for inpatient and outpatient, and specialty clinics in the following areas: geriatric psychiatry, frailty syndrome, urinary incontinence, memory disorders, falls and mobility disorders, and wound care. There are also community-based programs including a home-based primary care program and a Program of All-inclusive Care for Elders (PACE).
