4. An official record of board scores (NBME, FLEX, USMLE, LMCC, VQE, or FMGEMS)
5. Three letters of recommendation
6. Updated curriculum vitae
7. A personal statement
8. A valid ECFMG certificate (if a graduate from medical school outside of the United States or Canada)
6. A letter from the anesthesiology residency program director confirming successful completion of residency training
