If you are the donor, complete the Dedication, Cremated Remains, and Vitals Statistics forms.
If you are the next of kin completing for the donor, complete the Declaration of Consent, Cremated Remains, and Vital Statistics forms.
The Supplemental Information and Consent for Specific Procedures forms are voluntary.
Fill in, print, sign, and return via email, mail, or fax.
If emailing the forms, send to anatboard@med.miami.edu. In the subject line, please state if paperwork is for a future signup, or a near (imminent) death.
If you wish, make a copy for your records.
