Body Donation

1. Complete the Dedication Form, Cremated Remains Form, and Vital Statistics Form
2. The signed and completed forms can be scanned and emailed to anatboard@med.miami.edu, FAXed to 305-545-7166, or can be mailed to:

3. Instruct your next of kin, or individual handling your affairs after death to complete the steps under “Instructions for Final Arrangements”.
4. If you are going to do a pre-arrangement with the funeral home the funeral director should be informed of your plans to donate your body for medical education. They can contact the Miami Branch of the Anatomical Board.
5. If you wish further information about your donation or the completion of these forms, please call or email the Miami Branch of the Anatomical Board.

1. Make arrangements with a funeral home of your choice. Inform them that the loved one is a donation to the University of Miami Willed Body Program. They should be instructed to embalm the body by means of ‘arterial injection only’.
   
   
2. Instruct the funeral home if they have any questions, to contact us at 305 243 6691. Within a reasonable time after death, the Anatomical Board should be notified by the funeral home at the following:

3. The cost of the embalming and transportation to the Medical School will be borne by the family or estate of the deceased.
4. If cremation is not desired, the Anatomical Board should be informed. After study, the body can be turned over to a funeral home for alternative disposition methods which the family specify such as burial. The expenses of this option will be borne by the family.
