Training Verification Miller School of Medicine

In order to provide a uniform process for verifying residency/fellowship training for credentialing purposes, the Sponsoring Institution has adopted a standard Verification of Graduate Medical Education and Training (VGMET) form. The Verification Policy applies to all graduate medical education training programs at the University of Miami/JFK Medical Center Palm Beach Regional GME Consortium.

Residency/fellowship training verification forms are processed by the individual programs. Listed below are the University of Miami/JFK Medical Center Palm Beach Regional GME Consortium programs and their direct contact information.

All requests MUST be accompanied by the resident’s signed authorization to release information.

All requests should come directly from the entity requesting the verification, with the exception of state licensing forms.

**NOTE: VGMETs are completed following a resident’s successful completion of the program. Therefore, they could take up to three weeks following the end of the academic year on June 30th to be available for release.

State Licensing Boards:

Training verification forms for State Licensing Boards or FCVS will be completed separately from the VGMET form.

Verifications received directly from state licensing boards are exempt from the administrative service charge.

Case Logs

Trainees who have completed the following programs at the University of Miami/JFK Medical Center Palm Beach Regional GME Consortium in Atlantis, Florida (ACGME program no. 1401131535) do not create case logs during training:

Hospice & Palliative Care (1 year)
Internal Medicine: Preliminary Training (1 year)
Internal Medicine: Categorical Training (3 years)
Internal Medicine: Chief Residency (1 year)

The ACGME does not require case logs to be maintained for any of the above programs. Case logs for the following training programs will be included with the complementary institutional VGMET:

Cardiovascular Disease (3 years)
General Surgery: Preliminary Training (1 year)
General Surgery: Categorical Training (5 years)

Income Verification

UM residents and employees can now provide The Work Number website (www.theworknumber.com) when applying for a loan, buying a car, leasing an apartment, or similar instances where proof of employment or income is needed. The service is available 24 hours a day, seven days per week, making it faster for University employees to have their income verified for credit rapidly and easily, even on weekends or during evening hours.

Malpractice Verification

The Graduate Medical Education Office does not process malpractice verification forms. In order to obtain claims history and/or a certificate of insurance, please contact the University of Miami Risk Management office via email at riskmanagement@miami.edu. The office can be reached at 305-284-3163, however, requests are only accepted in writing.
Diploma Replacement Request

Duplicate diplomas may be requested by filling out a Diploma Request Form at a $25.00 charge per diploma plus shipping fees payable by check or money order.
Bio/Demo Change Request
Trainees who need to update or correct their name must request this change in writing and provide documentation to the University of Miami Graduate Medical Education Offices.

- Birth Certificate (This can only be used for students who are asking to have spelling errors in their given name corrected. To change a married name back to a maiden name, legal documentation other than birth certificate must be provided.)
Training Verification

Milestone Evaluations: For Programs

Final summative milestone evaluations can be downloaded directly from the ACGME via the ADS portal by any ACGME training program receiving a University of Miami trainee. Evaluations should be available after the trainee’s final milestone reporting period has ended.

Milestone Evaluations: For Trainees/Alumni

ADS has launched a beta version of a new tool for residents/fellows to retrieve their Milestone evaluations and the program-level aggregated results of their Resident/Fellow Survey reports. The Resident/Fellow Portal will also allow users the ability to edit some of their personal information.

To create their Resident/Fellow Portal account, individual residents and fellows should visit https://apps.acgme.org/ads/public/residentportal/search, or click Resident/Fellow Portal from the ADS login page, and enter their relevant information.
