This is a randomized, placebo-controlled, multi-center study. The study includes a Screening Period of up to 12 weeks. After the Screening Period, subjects will be randomly assigned in a 1:1 ratio to receive either paltusotine or placebo. At the end of the Treatment Period, subjects, who in the opinion of the Investigator, may benefit from treatment with paltusotine, may be enrolled in a long-term open-label extension (OLE) for up to 96 weeks. During the OLE, all subjects will receive paltusotine.

This Phase 2 study aims to evaluate the safety and efficacy of IMVT-1402, a novel FcRn inhibitor, in patients with Graves' disease who have elevated thyroid-stimulating hormone receptor (TSHR) antibodies. The scientific rationale is based on IMVT-1402's mechanism of blocking FcRn-mediated recycling of IgG autoantibodies, as well as previously demonstrated proof-of-concept data from batoclimab, a similar FcRn inhibitor. The study will enroll patients with confirmed Graves' disease who are either overtly or subclinically hyperthyroid and have significantly elevated TSHR antibodies (3× ULN), a population in which reducing pathogenic autoantibodies could potentially modify disease progression.
